Job summary

Navigo Health and Social Care CIC is recruiting a Rehabilitation Support Worker to join our multidisciplinary team. The role focuses on spending time with individuals and delivering short and long term reablement to help them regain independence.

Working across community and inpatient settings, you will support daily living activities, provide 1:1 and group interventions, document progress, and collaborate with therapists, nurses and social care colleagues to promote recovery.

Qualifications

• 3 years experience of working with people with mental illness.
• A good practical understanding of the MHA and MCA.
• Lived Experience.
• Mental Health First Aid training.
• Awareness of Health and Safety in the workplace.
• Recent and/or current work in Mental Health or Social Care.

Responsibilities

• Develop knowledge of lived experience to tailor care and support recovery.
• Advocate for service users to design their own mental healthcare.
• Build supportive relationships and provide daily encouragement.
• Deliver prescribed 1:1 and group interventions within the care plan.
• Assist with budgeting, welfare rights, planning and meals.
• Support education, occupation, training and development.
• Promote healthy lifestyle and daily living activities.
• Administer oral medication as required.
• Provide daily mood, behaviour, and progress updates to senior staff.
• Maintain electronic records and work across pathways as needed.
