Job summary

Surrey Downs Health and Care is seeking a Senior Rehab Support Worker to join the Recovery at Home team, delivering care and rehabilitation in community and hospital settings across Epsom area. You will assess clients, develop care plans and work with a multidisciplinary team to support timely discharge and home-based assessments.

The post requires NVQ level 3 in Health/Social Care or equivalent, good IT skills, and a valid UK driving license.

Qualifications

• Educated to NVQ 3 level or equivalent in a relevant subject.
• UK full driving license and access to a car with business insurance.
• Intermediate knowledge of IT systems (Outlook, Word, Excel).
• Experience in NHS, social care or related sector is desirable.

Responsibilities

• Complete skilled screening assessment for referred clients.
• Carry a delegated caseload within defined clinical area and plan treatments.
• Undertake skilled therapeutic rehabilitation and report progress to MDT.
• Work independently implementing and evaluating OT/physiotherapy interventions.
• Provide feedback to MDT and document patient condition changes.
• Assist MDT in evaluating client intervention and progress.
• Identify equipment needs and adaptions to promote independence.
• Deliver planned therapeutic activities and group sessions as needed.
• Perform risk assessments and continually evaluate safety.
