Job summary

Care Plus Group is seeking a Rehabilitation Support Worker to help people stay independent at home and support discharge from hospital. You will work with a wide range of professionals to enable users to regain confidence and skills, providing person-centered care and medication administration in line with policy.

Qualifications

• Ability to effectively communicate with service users, families and colleagues.
• Demonstrated empathy and person-centered approach to care.
• Willingness to work as part of a multidisciplinary team.

Responsibilities

• Provide short-term enablement and support to service users at home.
• Collaborate with physiotherapists, occupational therapists, nurses and care managers to maximise independence.
• Assist with assessment-driven care and respond flexibly to changing needs.
• Administer medications in accordance with policy and maintain accurate records.
