Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a Band 4 Senior Rehabilitation Assistant to join a multidisciplinary team delivering rehabilitation programmes. You will support physiotherapists and occupational therapists in care homes and patients’ own homes, helping people regain independence and coordinate sessions.

In this part-time permanent role, you will manage caseloads, contribute to service development and attend peer meetings with Band 4 staff, under guidance

Qualifications

• Level 2 Gym instructor qualification or equivalent.
• Level 3 or 4 Personal Training Diploma or equivalent.
• Member of the Register of Exercise Professionals (REPs) or equivalent.
• Full UK Driving Licence required at time of application.
• Appropriate Liability Insurance or Otego qualification.
• Experience delivering community balance and falls prevention classes.

Responsabilités

• Coordinate and deliver rehabilitation programmes (1:1 and group) in care homes and patients' homes.
• Collaborate with physiotherapists and occupational therapists to rehabilitate patients with mixed diagnoses and escalate concerns.
• Identify signs of patient deterioration and refer back to therapists as needed.
• Manage diary for classes/patients.
• Provide training to wider rehabilitation workforce on safe exercise rehabilitation.
