Job summary

NELFT NHS Foundation Trust is seeking a motivated Rehabilitation Assistant (Band 4) to support Falls Strength and Balance services across West and East Essex. You will deliver patient-centred assessments, group and home-based exercise sessions, and contribute to care planning and service development.

The role requires a Postural Stability Qualification and experience running community-based strength and balance classes, with a focus on preventing falls in more independent patients.

Qualifications

• Postural Stability qualification and experience delivering strength and balance classes.
• Experience running home-based exercise programmes to support independence.
• Caseload management in community rehabilitation settings.

Responsibilities

• Triaging referrals and determining patient eligibility for falls programmes.
• Conducting initial home assessments to decide Otago or Postural Stability options.
• Leading and delivering structured strength and balance sessions (home-based or group).
• Supporting development, evaluation and improvement of service commitments.
