Job summary

Hertfordshire County Council is seeking a Senior Occupational Therapist to join the Integrated Discharge Service in West Hertfordshire. You will deliver strengths-based assessments and develop person-centred support plans to promote independence following hospital discharge.

Working within a multidisciplinary team, you will support Home First and Discharge to Assess pathways, provide OT interventions, equipment recommendations, and contribute to safeguarding and integrated health and social care

Qualifications

• HCPC registration as an Occupational Therapist and a minimum of two years' post-qualification OT experience.
• Experience of completing assessments and making recommendations to support independence and wellbeing.
• Knowledge and experience of working within the social care sector.

Responsibilities

• Provide occupational therapy advice and interventions, including equipment, adaptations and strategies to enable participation in daily activities.
• Develop strengths-based assessments and person-centred support plans to promote independence and wellbeing.
• Support Home First and Discharge to Assess pathways within a multi-disciplinary team.
