Our Integrated Discharge Service in West Hertfordshire plays a vital role in supporting people through their hospital discharge journey. Working within a collaborative, multi-disciplinary environment, we focus on ensuring individuals receive the right support at the right time to maximise their independence and wellbeing.
As part of the Post Hospital Review Team, you will work alongside Social Workers, Occupational Therapists, Community Care Officers, health professionals and care providers, supporting both Home First and Discharge to Assess pathways. As an Occupational Therapist within the Integrated Discharge Service, you will work with residents, their families and partner agencies to deliver strengths-based assessments and develop person-centred support plans that promote independence and wellbeing. Working as part of a multi-disciplinary team, you will support individuals through the Home First and Discharge to Assess pathways, helping them achieve safe and positive outcomes following hospital discharge.
You will provide occupational therapy advice and interventions, including recommendations for equipment, adaptations and strategies that enable people to participate confidently in everyday activities. You will also contribute to a preventative approach that reduces the need for longer-term support, build effective links with community resources, and support colleagues within the wider team. There will also be opportunities to develop a specialist area of practice and further enhance your professional skills and expertise.
We're looking for a motivated Occupational Therapist who is passionate about supporting people to live as independently as possible.
