Job summary

Connect2Dorset seeks an experienced Occupational Therapist to support the Home First Social Care Team across Dorset. The role focuses on hospital discharge, active recovery and reablement, delivering integrated care with health and community partners.

The post requires UK training, HCPC registration, and a driving licence with willingness to travel across the county. Office-based at County Hall, Dorchester with options for home working where appropriate.

Qualifications

• Enhanced DBS (we can obtain for you)
• Eligibility to work in the UK
• SWE (HCPC) Registered
• Degree in Occupational Therapy
• 3 years post qualified experience within the UK
• Have access to a car and be willing to travel
• Knowledge and experience in applying relevant frameworks and theories

Responsibilities

• Support hospital discharges, active recovery, and reablement packages.
• Work with partners across health and community services to collaborate effectively.
• Travel across the county to visit clients as needed.
• Be based at County Hall, Dorchester with hybrid/office-based work when appropriate.
