Job summary

Sheffield Teaching Hospitals NHS Foundation Trust is seeking an experienced Occupational Therapist to lead complex discharge planning within the Care Transfer Hub. You will coordinate multi-agency input, support staff with advanced clinical reasoning, and drive improvements in intermediate care pathways.

The role requires extensive OT experience, HCPC registration, and a track record in management, education, and service development within NHS discharge services.

Qualifications

• BSc in Occupational Therapy or equivalent.
• Current HCPC Registration.
• Postgraduate Masters level module or equivalent knowledge through training.
• Evidence of continuous personal and professional development.
• Knowledge of discharge and intermediate care evidence-base.
• Up to date NHS policy initiatives and developments.

Responsibilities

• Lead high-quality therapy care in a defined clinical pathway.
• Provide expert clinical practice with advanced reasoning and support to staff.
• Offer clinical leadership to develop staff and practice.
• Support service managers in improving AHP services and discharge pathways.
