Job summary

Hertfordshire County Council is seeking a Senior Occupational Therapist to join the Post Hospital Review Team in Hemel Hempstead. You will assess and support adults transitioning from hospital to community, delivering person-centred plans to promote independence and wellbeing.

Working within a multi-disciplinary Integrated Discharge Service, you will provide OT advice, assess equipment or home adaptations, and contribute to safeguarding and joined-up health and social care pathways.

Qualifications

• HCPC registration as an Occupational Therapist with post-qualification experience.
• Experience completing assessments to support independence.
• Knowledge of the social care sector.
• Full driving licence and access to a vehicle.
• Knowledge of Care Act 2014 and Mental Capacity Act 2005.

Salary:£44,253 plus market forces payment of £2,326, progressing to £48,738 plus a market enhancement of £2,170 – pro rata per annum

Our Integrated Discharge Service in West Hertfordshire plays a vital role in supporting people through their hospital discharge journey. Working within a collaborative, multi-disciplinary environment, we focus on ensuring individuals receive the right support at the right time to maximise their independence and wellbeing.

As part of the Post Hospital Review Team, you will work alongside Social Workers, Occupational Therapists, Community Care Officers, health professionals and care providers, supporting both Home First and Discharge to Assess pathways. About the role

As an Occupational Therapist within the Integrated Discharge Service, you will work with residents, their families and partner agencies to deliver strengths-based assessments and develop person-centred support plans that promote independence and wellbeing. Working as part of a multi-disciplinary team, you will support individuals through the Home First and Discharge to Assess pathways, helping them achieve safe and positive outcomes following hospital discharge.

You will provide occupational therapy advice and interventions, including recommendations for equipment, adaptations and strategies that enable people to participate confidently in everyday activities. You will also contribute to a preventative approach that reduces the need for longer-term support, build effective links with community resources, and support colleagues within the wider team. There will also be opportunities to develop a specialist area of practice and further enhance your professional skills and expertise.

About you

• HCPC registration as an Occupational Therapist and a minimum of two years' post-qualification Occupational Therapy experience.
• Experience of completing assessments and making recommendations to support individuals' independence and wellbeing.
• Knowledge and experience of working within the social care sector.
• The ability to work independently while contributing effectively as part of a wider team.
• A full driving licence and regular access to a vehicle.
• Knowledge of the Care Act 2014.
• Knowledge of the Mental Capacity Act 2005.
