Job summary

Senior Occupational Therapist in the Post Hospital Review Team at Hertfordshire County Council, Hemel Hempstead, will support patients through hospital discharge by delivering strengths-based assessments and person-centred care to maximise independence.

In a multi-disciplinary Integrated Discharge Service, you will contribute to Home First and Discharge to Assess pathways, providing OT advice on equipment, adaptations and strategies for daily living while collaborating across health and social

Qualifications

• HCPC registration as an Occupational Therapist and a minimum of two years' post-qualification Occupational Therapy experience.
• Experience of completing assessments and making recommendations to support individuals' independence and wellbeing.
• Knowledge and experience of working within the social care sector.
• The ability to work independently while contributing effectively as part of a wider team.
• A full driving licence and regular access to a vehicle.
