Qualifications

Job summary

An exciting opportunity has arisen to recruit a Band 6 Senior Nurse Practitioner to join an established nursing team within the Greater Manchester Specialist Support Team (GMSST). This team provides a service across Greater Manchester to service users with learning disabilities and/or autism who are at risk of hospital admission, placement breakdown, or forensic risk.

The post is based in Eccles and involves working within a multidisciplinary team that includes Nursing, Occupational Therapy, Social Work, Psychology, Speech and Language Therapy, and Support Workers. You will work across Greater Manchester, liaising with local Learning Disability, Mental Health, and Autism services to develop proactive risk mitigation, discharge support for inpatients, and preventative strategies.

Main duties include:

  1. Providing high-quality specialist assessments and interventions for individuals with complex needs and challenging behaviors.
  2. Managing your own caseload, including risk assessments and developing risk management plans.
  3. Supporting the implementation of therapeutic interventions and evaluating their outcomes.
  4. Developing discharge plans and service specifications.
  5. Disseminating the Positive Behavioural Support model and advising staff teams.
  6. Occasionally undertaking urgent assessments and contributing to crisis management plans.
  7. Leading on supervision, mentoring, and training of staff and students.
  8. Facilitating interagency communication and ensuring safeguarding standards are maintained.
  9. Engaging in research, audits, and service development activities.

Essential qualifications and skills:

  • First-level Nurse Registration in Learning Disability or Mental Health.
  • Degree/Diploma or equivalent experience/knowledge.
  • Excellent communication skills, ability to work independently and in teams, and experience with complex behavior assessments.
  • Driving Licence.

Essential knowledge and experience:

  • At least 18 months post-registration experience working with individuals with learning disabilities/autism who have offended or display challenging behaviors.
  • Experience in forensic risk assessment, legal frameworks, and interagency working.

Additional requirements:

  • Current UK professional registration.
  • Disclosure and Barring Service (DBS) check.
