Job summary

Practice Plus Group is seeking a Senior Mental Health Practitioner for the Mental Health team at HMP Stocken (Stocken, Rutland). You will join a multi-disciplinary team, supporting a full-time schedule with 37.5 hours per week across 8am–5pm shifts and a 1-in-4 weekend pattern.

The role offers a salary between £43,609 and £49,175 per year, depending on experience. You will assess and manage patients’ mental health needs, work autonomously within a team, and contribute to a holistic care plan

Qualifications

• NMC registered in Mental Health field.
• Experience working with neurodiverse conditions incl. autism/ADHD.
• Proven ability to work in a multi-disciplinary setting.

Responsibilities

• Responsible for assessment and ongoing management of patients' mental health needs.
• Autonomously manage a caseload within a multi-disciplinary team to develop holistic care plans.
• Deliver evidence-based care and maintain therapeutic relationships with patients.
• Contribute to nursing strategy and ensure care is personalised and aligned with what matters to patients.
• Participate in safeguarding and follow policies during assessment, planning, implementation and evaluation.

The Mental Health Service is covered

• Monday to Sunday service
• 37.5 hours per week
• Various shift patterns covering 8am - 5pm
• 1 weekend in 4

This position is full time, you’ll receive an annual salary between £43,609- £49,175 per annum - depending on experience.

What you'll be doing

• Responsible for assessment and ongoing management of our patients’ mental health needs.
• Work collaboratively with the multi-disciplinary team as an autonomous practitioner managing a caseload of patients, assessing their physical, social and psychological needs to develop a holistic health care plan.
• Contribute to our nursing strategy and vision to; Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
• Deliver non-judgemental care, building therapeutic relationships and promoting professional boundaries.
• Play an active part in the assessment, planning, implementation and evaluation of the patient’s care following our policies and procedures and always considering safeguarding.
