Job summary

Practice Plus Group is recruiting a Senior Primary Care Nurse to join the Primary Care team at HMP Wymott, a Category C male prison in Leyland. The role involves leading a multidisciplinary team, conducting clinics, responding to emergencies and managing medication rounds within a prison healthcare setting.

You will work across prison wings and be supported by a bespoke induction, competency framework and career development opportunities.

Qualifications

• Registered nurse with Adult NMC registration and CPD evidence.
• Understanding of multi-disciplinary and multi-agency working.
• Excellent communication skills and proactive problem solving.

Responsibilities

• Lead a multi-disciplinary team to deliver a nurse-led service and prevent hospital admissions.
• Undertake medication rounds and attend emergency response situations.
• Dispense controlled medication and assess patients with a focus on personalised care.
• Provide non-judgemental care and build trusted relationships with patients.

We have an exciting opportunity for a Senior Primary Care Nurse to join our Primary Care team at HMP Wymott, a Category C male prison located in Leyland. Ideally needs to be a car driver due to location of the prison

HMP Wymott has capacity of around 1,100, and has a varied and complex population.

You will have experience in Primary Care and will be responsible for conducting general clinics, responding to emergencies, and completing medication rounds.

You will work as part of a multidisciplinary team, based within the healthcare department, and will also be required to work across the prison wings.

Hours:

• Monday - Sunday service
• 37.5 hours per week
• 7.30am - 9.00pm
• Every other weekend
• Very occasional nights

Salary:

This position is full time, you’ll receive an annual salary up to £43,609 - £49,175 per annum - depending on experience.

What you’ll be doing

• Lead a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
• Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
• Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
