Portsmouth Hospitals University NHS Trust is seeking a Band 5 Medicines Management Technician to support the ward-based medicines management service. You will perform medicines reconciliation, assess PODs, and assist in discharge medication checks while counselling patients on their medications.

The role includes developing and delivering enhanced medicines management services and providing training to staff.

Qualifications

• Must be UK registered as a pharmacy technician with the GPhC.
• Experience with medicines reconciliation and POD assessment.
• Hospital setting experience desirable.
• Ability to train and supervise others.
• Commitment to CPD and professional development.

Responsabilités

• Support medicines reconciliation by confirming and documenting medication taken by patients pre-admission.
• Assess PODs for extended ward use and discharge medicines.
• Resolve discrepancies or interventions with patients' drug history.
• Coordinate discharge plans and prioritise prescription supply with ward staff.
• Dispense or check medicines for inpatients and TTOs.
• Address compliance/concordance issues with other healthcare professionals.
