Job summary

Portsmouth Hospitals University NHS Trust is seeking an NHS Band 5 pharmacist support role to work with Clinical Pharmacists on medicines management. You will perform medicines reconciliation, POD assessment, discharge checks, and patient counselling to ensure high standards of care.

The role involves training and supervising pharmacy technicians and pre-registration graduates, with opportunities to develop enhanced medicines management services and training materials.

Qualifications

• GCSE English, Maths, Science and 2 other GCSEs or equivalent.
• BTEC (Pharmaceutical Science) or NVQ level 3 (Pharmaceutical Science).
• Registered as a pharmacy technician with the General Pharmaceutical Council (GPhC).
• Recognised Accredited Checking Technician Qualification.
• Foundation Degree in Medicines Management.
• Accredited facilitator for ACPT, MMT or communication skills.
• NVQ assessor.

Responsibilities

• Assist medicines reconciliation and documentation per Medicines Reconciliation policy.
• Assess suitability of PODs for ward use and discharge.
• Resolve discrepancies in patient drug history with the ward team.
• Support discharge plans and ensure timely prescription supply.
• Dispense or check inpatients and TTOs per legal/ethical standards.
• Provide patient counselling on medicines use.
