Job summary

The Bank Senior HCA role is part of our Community Services team, providing high-quality palliative and end-of-life care to patients and families in their homes. You will support personal care, emotional wellbeing, and practical needs, and may deliver respite care to relieve families during challenging times.

You will work in partnership with NHS colleagues and local partners to respect patient wishes to remain at home, contributing to wellbeing-focused care and ensuring timely home-based support

• Resilience and ability to manage time effectively.
• Ability to work independently and as part of a team.

Responsibilities

• Provide personal care, emotional wellbeing and practical support to patients at home.
• Deliver periods of respite care for families and carers.
• Collaborate with NHS, social services, local care homes and voluntary organisations.
• Support palliative and end-of-life care in the Hospice at Home and community settings.
• Work with the Rapid Response team to deliver urgent care when needed.
