Job summary

Trinity Hospice provides compassionate, person-centred care for patients in the last year of life, delivered in patients' homes by a dedicated Community Nursing Team. The Health Care Assistant will support admissions, discharges and ongoing care packages, and help with daily living tasks while ensuring dignity and comfort.

Qualifications

• Compassionate and patient-focused care is essential.
• Able to communicate effectively with patients, families and colleagues.
• Experience in a health care setting preferred.
• Adequate IT skills (email/word) and ability to maintain records.

Responsibilities

• Deliver high-quality personal and practical care to patients in their own homes.
• Support admissions, discharges and ongoing care packages.
• Assist with washing, dressing, mobility and continence care.
• Monitor patient wellbeing and report changes to the nurse in charge.
• Provide comfort and support to patients, families and carers.
