St Nicholas Hospice Care is seeking a dedicated Health Care Assistant to provide direct patient care and support to patients and families in our hospice and community settings.

You will work with registered health professionals to deliver compassionate palliative care, assist in care planning, and uphold confidentiality while supporting emotional and spiritual wellbeing. The role requires empathy, teamwork, and commitment to mandatory training and safety policies.

Qualificações

Responsabilidades

• Carry out tasks involving direct patient care and support to patients and families.
• Assist in assessment of individuals' needs arising from health and social status.
• Support carers to cope with emotional, psychological and spiritual wellbeing.
• Contribute to care planning and review by implementing, monitoring and evaluating care.
