Job summary

Surrey and Borders Partnership NHS Foundation Trust is seeking a Complex Case Pathway Lead to coordinate discharge planning for high-risk mental health service users. The role focuses on forming discharge pathways with partners before or at admission to prevent unnecessarily long hospital stays.

Candidates should have seven years of mental health experience, including two years at a senior level, and a specialism in forensics, CYPS, autism or complex case management.

Qualifications

• Seven years' experience in a mental health environment, including two years at a senior level.
• Specialism in forensics, CYPS, autism or complex case management.
• Experience identifying and interpreting national policy.
• Evidence of continued professional development.

Responsibilities

• Manage the caseload of service users admitted to or awaiting admission to acute mental health beds at high risk of long length of stay.
• Develop links with system partners and internal and external stakeholders.
• Facilitate a discharge pathway before or on admission to avoid unnecessary length of stay.
• Achieve the best clinical and financial outcome for each individual.
