Responsibilities

• Manage caseload of service users admitted to or awaiting admission to acute mental health beds and high risk of Long Length of Stay (LLOS) or restrictive discharge outcomes.
• Develop links with system partners and internal/external stakeholders to enable discharge pathways before or upon admission to acute mental health beds.
• Collaborate to avoid unnecessary length of stay and ensure best clinical and financial outcomes.
• Contribute to continuous improvement of discharge processes and pathways.

Job description

The Complex Case Pathway Lead will be responsible for managing the caseload of service users being admitted to or awaiting admission to acute mental health beds who are at high risk of a Long Length of Stay (LLOS) and restrictive discharge outcomes due to complex needs.

The Complex Case pathway lead will be expected to develop links with system partners and other internal/external stakeholders to facilitate a discharge pathway for these individuals prior to/upon admission to an acute mental health bed to avoid unnecessary LOS and ensure the best outcome from a clinical and financial perspective., Please See attached job Description and person specification that details the main duties of the job within
