Job summary

Qualifications

• Registered Mental Health or Learning Disability Nurse or other registered health/social care professional.

Responsibilities

• Manage the caseload of service users admitted to or awaiting admission to acute mental health beds who are high risk for Long Length of Stay and restrictive outcomes.
• Develop links with system partners and internal/external stakeholders to facilitate discharge pathways prior to or upon admission.
