Qualifications

• Qualified Health or Social Care Practitioner with current registration.
• Evidence of continuing professional development and post-qualification experience.

Responsibilities

• Lead daily board rounds and provide expert clinical guidance for complex discharges.
• Coordinate and manage complex discharges including CHC, NRPF, and out-of-area cases.
• Provide clinical supervision and coaching to IDS Coordinators.
• Ensure timely decision-making and accurate documentation in CareFlow.
