Job summary

Surrey and Borders Partnership NHS Foundation Trust is seeking a Complex Case Pathway Lead to manage the caseload of service users being admitted to or awaiting admission to acute mental health beds at high risk of long length of stay due to complex needs.

You will develop links with system partners to facilitate a discharge pathway before or upon admission, with a focus on safe, compliant, and financially sound outcomes.

Qualifications

• 7 years' experience in a mental health environment with 2 years at a senior level.
• Evidence of continued professional development.
• Knowledge of the wider system.

Responsibilities

• Manage caseload of service users being admitted to or awaiting admission to acute mental health beds.
• Develop links with system partners and other internal/external stakeholders to facilitate discharge pathways.
• Aim to avoid unnecessary long length of stay and ensure best clinical and financial outcomes.
