BCUHB North Wales is seeking a Senior Complex Case Adviser, an experienced Registered Nurse or Allied Health Professional. You will lead the Complex Case Advisory Service, develop bespoke care pathways, and work with health, social care, and third sector partners to ensure timely, person-centred care.

The role focuses on complex discharge planning, safeguarding, Mental Capacity, and Court of Protection matters, with leadership, coaching, and collaboration across organisations.

Competenze

• Registered nurse 1st Level or AHP
• Evidence of ongoing professional development and training
• Degree or Diploma Registered nurse 1st Level
• NMC Registration or AHP registration
• Postgraduate diploma or equivalent level experience

Mansioni

• Provide expert advice and support for patients with highly complex health and social care needs.
• Lead on complex discharge planning and pathway development.
• Support clinical teams in challenging situations involving safeguarding and Mental Capacity, Best Interest decisions, and Court of Protection matters.
• Work collaboratively across organisational boundaries to facilitate safe, person-centred care.
• Contribute to reducing unnecessary delays in discharge and improving patient outcomes.
• Support monitoring of high-cost packages of care and specialist commissioned placements.
• Provide leadership, coaching and professional support to colleagues.
