• Evidence of ongoing professional development and training.
• NMC Registration or AHP registration.

Aufgaben

• Provide expert advice and leadership for patients with highly complex needs.
• Lead on complex discharge planning and pathway development.
• Support clinical teams in challenging situations including Mental Capacity, Best Interest and safeguarding matters.
• Collaborate across organisational boundaries to enable safe, person-centred care.
• Contribute to reducing delays in discharge and improve patient outcomes.
• Monitor high-cost packages of care and specialist placements.
• Provide leadership, coaching and professional support within the service.
