Job summary

Leaders in Care is seeking a Complex Care Coordinator in Wolverhampton to support specialist community care packages across the West Midlands. You’ll blend rota coordination with day-to-day operations, including field visits and staff supervision.

This hands-on role suits someone with care coordination experience, strong organisational skills and the ability to adapt in a fast-paced setting. A full UK driving licence and access to your own vehicle are essential for regular field visits.

Qualifications

• Previous experience in care coordination or field care supervision.
• Experience in complex or domiciliary care beneficial.
• Good understanding of CQC requirements and person-centred care.
• Strong rota management and organisational skills.
• Confidence communicating with care staff, clients, families and healthcare professionals.
• Full UK driving licence and access to own vehicle for field visits.

Responsibilities

• Create and manage staff rotas to ensure care packages are covered.
• Coordinate with clients, families, carers and operations team.
• Manage last-minute staffing changes and resolve issues quickly.
• Conduct staff supervisions, appraisals and team meetings.
• Support onboarding and compliance of new care staff.
• Carry out regular field visits across the West Midlands.
• Attend MDT meetings and client reviews with clinical and operational colleagues.
• Support safeguarding, incident reporting and continuous service improvement.
• Liaise with healthcare professionals and commissioners.
• Participate in on-call rota (one weekday evening & approx. one weekend every 3–4 weeks).
