Job summary

Leaders in Care seeks a Complex Care Manager to oversee community-based packages for adults and children with complex needs. You will set up new services, ensure high standards, and work with clinicians, families and commissioners across the West Midlands and Shropshire.

You’ll lead care teams, supervise staff, and drive service improvement while managing on-call and rota coverage, with Autonomy to grow your own services.

Qualifications

• Strong experience within community or domiciliary care.
• Management or senior leadership experience within complex care.
• Hands-on knowledge of complex clinical packages (ventilation, tracheostomy, spinal injury, acquired brain injury or bowel care).
• Experience supporting complex care packages for adults and/or children.
• Knowledge of CQC requirements and person-centred care.
• Confidence working with families, clinicians, commissioners and MDTs.
• Strong leadership and problem-solving skills.
• Full UK driving licence and access to your own vehicle.
• Willingness to travel across the West Midlands and Shropshire.

Responsibilities

• Carrying out regular client visits across the West Midlands and Shropshire.
• Completing and signing off person-centred care plans.
• Working closely with clinicians and MDTs to review complex care packages.
• Building relationships with ICBs, local authorities and other commissioners.
• Preparing and submitting reports to commissioners.
• Leading, supporting and supervising care teams.
• Completing staff supervisions, appraisals and team meetings.
• Supporting rota management and ensuring packages remain appropriately staffed.
• Supporting safeguarding, incident management and service improvement.
• Participating in the on-call management rota.

Are you an experienced Complex Care Manager looking for a role where you can take real ownership of community-based care packages?

We are looking for a Complex Care Manager to oversee the delivery of specialist care packages across the West Midlands and Shropshire, supporting both adults and children with a range of complex clinical needs.

You'll take responsibility for setting up new packages, maintaining existing services and working closely with clients, families, clinicians and commissioners to ensure consistently high standards of care.
