The Place Care is recruiting an experienced Care Coordinator to lead the coordination of complex home-care packages across London. The role requires building strong links with families, staff and healthcare professionals to ensure safe and effective delivery of care.

We offer a competitive salary in the £33,000–£36,000 range and a company vehicle for work-related travel. A full UK driving licence is essential for this role.

Qualifications

• Experience coordinating complex care packages.
• Experience in domiciliary care, home care or supported living.
• Ability to coordinate staff rotas and ensure coverage.

Responsibilities

• Coordinate complex care packages and ensure staffing is maintained.
• Prepare and manage staff rotas to ensure continuity of care.
• Match care staff to service users according to needs and risks.
• Ensure shifts are covered and respond promptly to sickness or changes.
• Maintain effective communication with service users, families and professionals.
• Support development and review of person-centred care plans and risk assessments.
• Travel between service-user locations to monitor care quality.
