Leaders In Care Ltd in Rochdale is seeking a proactive Care Coordinator to join our growing team supporting complex care clients across the North West. You will coordinate care packages and work closely with HCAs and nurses to ensure high-quality, person-centred care.

Qualifications

• Previous experience within care coordination, care management, domiciliary care or a similar role.
• Experience within complex care or community-based care would be advantageous.
• Excellent communication and relationship-building skills.

Responsibilities

• Coordinating care packages and ensuring staffing levels are maintained.
• Managing rotas and ensuring shifts are appropriately covered.
• Supporting recruitment, onboarding and allocation of HCAs and Nurses.
• Building strong relationships with clients, families and healthcare professionals.
• Carrying out regular staff and client check-ins.
• Supporting care plans and ensuring they remain accurate and up to date.
• Monitoring the quality of care and escalating concerns appropriately.
• Working closely with the wider operational and clinical teams.
• Supporting growth and development of new and existing care packages.
• Ensuring care is delivered in line with policies and regulatory requirements.
