Berkshire Healthcare NHS Foundation Trust in Bracknell is seeking a Band 6 Triage Clinician to join the Bracknell Community Nursing Team. The role combines office and home visits, providing clinical triage and immediate clinical advice to callers including healthcare professionals and patients.

You will work with clinical nursing staff to prioritise urgent cases, maintain professional registration, and ensure high-quality care in the community. A UK resident with access to a car is required.

Competenze

• NMC Current PIN.
• At least 2 years physical nursing experience within a community setting.
• Excellent critical thinking skills to make clinical decisions from caller information.

• Work as a key member of the Triage Team.
• Provide expert clinical advice that oversees entry into the service.
• Maintain up-to-date statutory and mandatory training.
• Hold professional registration and adhere to codes of practice.
• Collaborate with admin staff to provide high-quality clinical triage.
• Assist admin staff by reviewing cases requiring additional clinical advice.
• Provide urgent clinical support to patients at home and in-clinic settings.
• Prioritise tasks to ensure effective delivery during high demand.
