Berkshire Healthcare NHS Foundation Trust is seeking a Band 6 Triage Clinician to join the Bracknell Community Nursing Team. The role is both office and practice-based, providing clinical support to callers including patients and professionals, with visits to patients at home and occasional on-site clinic work.

The post requires an NMC PIN and at least 2 years of community nursing experience, plus strong critical thinking. A driving license is necessary to access patient homes.

Qualifikationen

• NMC Current PIN.
• At least 2 years physical nursing experience within a community setting.
• Excellent critical thinking skills to be able to make clinical decisions based on information callers provide.

Aufgaben

• Work as a key member of the Triage Team providing sound expert clinical advice.
• Maintain up-to-date Statutory and Mandatory training and rotate into clinical services as necessary.
• Hold professional registration and adhere to professional codes of practice and conduct.
• Collaborate with administrative staff and the community nursing service to provide high quality, clinical triage.
• Assist with transfer of cases requiring additional clinical advice and review recorded clinical information.
• Provide urgent clinical support to patients in their own homes and in-house clinics.
• Prioritise tasks to ensure effective delivery of services during high demand.
