Job summary

Qualifications

• Ability to work across multi-disciplinary teams in a fast-paced environment.

Responsibilities

• Signpost, refer and support access to out-of-hospital services and primary/secondary care.
• Identify urgent vs non-urgent referrals and allocate to appropriate pathways.
• Interrogate information systems to gather patient data for decision making.
• Collaborate with health professionals and carers to inform decisions.
