Job summary

Berkshire Healthcare NHS Foundation Trust is seeking a Band 6 Triage Clinician to join the Bracknell Community Nursing Team. The role combines office-based triage with patient visits in their homes and at clinics.

You will provide expert clinical advice, assess referrals, maintain training, and work with the wider nursing team to ensure timely, high-quality care for the Bracknell community. A valid UK driver's license and access to a car are required.

Qualifications

• Must hold NMC PIN and be registered to practice in the UK.
• Minimum 2 years of physical nursing experience in a community setting.
• Strong critical thinking skills to support clinical decisions.

Responsibilities

• Work as a key member of the Triage Team.
• Provide sound expert clinical advice guiding entry into the service.
• Maintain up-to-date statutory and mandatory training; rotate into clinical services as needed.
• Adhere to professional codes of practice and conduct.
• Collaborate with admin staff and the community nursing service to deliver high-quality clinical triage.
• Assist admin staff by transferring cases needing clinical advice and reviewing records.
• Provide urgent clinical support to patients at home and in-clinic settings.
• Prioritise calls effectively using questioning, active listening and clear communication.
• Coordinate with Clinical Nursing Staff to ensure continuity of care for callers.
