Job summary

Berkshire Healthcare NHS Foundation Trust in Reading is seeking a Band 6 Clinical Nurse Advisor to enhance the Community Nursing Triage service. The role requires a registered nurse with community experience, strong communication skills, and the ability to travel by car to patient visits and home care situations.

You will triage referrals, signpost to appropriate services, and provide expert clinical guidance to callers while maintaining compassionate patient contact and efficient service

Qualifications

• Registered nurse with community experience.
• Car driver with ability and readiness to travel.
• Excellent communication skills with the ability to manage distressed patients or relatives over the phone.
• Ability to use own initiative and work autonomously.

Responsibilities

• Provide clinical support, assistance, and professional expertise to all callers to the Community Nursing Triage Telephone Support, including signposting or referrals.
• Work with administrative staff to provide high quality clinical triage using knowledge, skills, and professional judgement.
• Control and manage calls effectively with skilled questioning, active listening, and clear verbal communication.
• Prioritise tasks and cases to ensure effective delivery during high demand periods.
• Visit urgent visits in the patient’s home, such as catheter care and end of life care.
