Job summary

Hampshire County Council's Reablement Team is hiring a Community Reablement Assistant (Level 2) to support adults over 18, helping them regain independence and stay at home. You will work with Occupational Therapists and Sensory Officers to deliver person-centred care and assist with reablement plans.

The role covers Winchester and surrounding areas, may include evenings and weekends, and requires a driving licence.

Qualifications

• Experience in social or health care settings.
• Ability to perform person-centred care and basic care tasks.
• Good interpersonal and communication skills.
• Dignity and respect in interactions.

Responsibilities

• Provide Person-Centred Care: initial home visits and reablement interventions to restore independence including personal care and moving and handling.
• Deliver Outcomes: risk assessments, update reablement plans and monitor progress for Level 1 Assistants.
• Support Operational Delivery: coach and mentor Community Reablement Assistants and assist team leads and therapists.
