Job summary

Hampshire County Council invites applications for a Community Reablement Assistant (Level 2) based at Dame Mary Fagan House, Basingstoke. You will support adults over 18 to maximise independence, helping them stay at home and coordinating with therapists and officers.

The role requires visiting homes, assessing needs and guiding reablement plans. Working patterns run 07:00-21:00, with 37 hours per week and a permanent or 6‑month fixed-term path.

Qualifications

• Understanding of the needs of service users, particularly in relation to reablement programmes.
• Previously worked within a social or health care setting.
• Role models our core values with patience, compassion, empathy and excellence in all interactions.
• Builds successful working relationships through strong interpersonal and communication skills.
• MO365 experience (including Word, Outlook and Excel).

Responsibilities

• Provide Person-Centred Care: Conduct initial home visits and independently perform reablement interventions to restore independence including intimate personal care and mobility tasks.
• Deliver Outcomes: Perform risk assessments, set goals, update reablement plans and monitor service user progress.
• Support Operational Delivery: Coach and mentor Community Reablement Assistants and assist team leaders and therapists.
