Our Integrated Neighbourhood Teams are made up of nurses, therapists, paramedics, and support workers who work together to support service users on their healthcare journey. We would love for you to come along for the ride.

Main duties of the job
  1. Complete holistic assessments of service users.
  2. Develop and deliver person-centered care plans.
  3. Line manage junior staff and contribute to their development through supervision, 1:1s, and training.
  4. Undertake and complete the appropriate training, supported by a robust competency framework, within the allocated time, to carry out the role and deliver competent clinical services.
  5. Communicate with the senior leadership team in the locality to ensure smooth service delivery.
  6. Identify risks and act accordingly to minimize risk to self, service users, and colleagues in line with organizational policies and procedures.
  7. Coordinate shifts and delegate/allocate care to the team in accordance with policy.
  8. Act as a point of escalation for staff to report issues or deteriorations.
  9. Undertake further training as required to develop extended skills and enhance patient care.

  • Ability to effectively lead a team with senior support when required
  • Ability to make clinically reasoned judgments working with various conditions
  • Experience supervising staff and students of own profession
  • Effective time and caseload management
  • Recent relevant community setting experience, including managing frail, complex patients
  • Experience supervising staff and students from other professions
