Qualifications

• Level one Registered General Nurse or Allied Health Care Professional.
• Postgraduate specialist training or masters level equivalent.
• Current and valid registration with the relevant professional body.
• Experience in managing complex long-term conditions and frailty.
• Independent Non-Medical Prescribing.

Responsibilities

• Triage patients to determine eligibility for the service.
• Holistic in-home assessment of clinical, physical, psychosocial and spiritual needs.
• Risk assessment to determine safest care setting and appropriate referrals.
• Medication assessment, review and safety in collaboration with prescribers.
• Case management planning and implementation to maximise independence.
• Maintain timely communication with patients, carers and services; participate in MDT meetings.
