Job summary

Qualifications

• Essential nursing qualification and registration as a nurse.
• Advanced assessment and prescribing qualifications are preferred.

Responsibilities

• Lead advanced clinical assessment and care planning for high-need patients.
• Provide clinical leadership and supervision within community nursing.
• Coordinate care with multi-professional teams and care homes.
• Work to reduce hospital admissions and improve patient outcomes.
