Job summary

Procare Community Services, within the Integrated Neighbourhood Team, seeks a Community Matron to manage complex caseloads, provide advanced clinical assessment, and coordinate care with GPs and partners. The role involves leading proactive case management and developing personalised care plans to reduce hospital admissions.

The post requires working rotating late shifts and weekends in the Community Coordination Centre, with eligibility to work in the UK and a current NMC Level 1 registration.

Qualifications

• MSc or Level 7 qualification including a health assessment module.
• Assessor, NMC supervisor or mentorship qualification.
• Current NMC registration as a Level 1 nurse.

Responsibilities

• Manage a caseload of people with complex needs and long-term conditions at high risk of unnecessary hospital admission.
• Provide advanced clinical assessment, diagnosis, treatment and evaluation of health needs.
• Provide expert clinical leadership and proactive case management.
• Develop personalised care plans that improve patient outcomes and experience.
• Coordinate holistic care with GPs, community services, social care, mental health teams and other partners.
• Support people to remain independent at home and reduce avoidable hospital admissions.
• Clinically review and prioritise new, complex and urgent referrals in the Community Coordination Centre.
• Work rotating late shifts and weekends to cover the Community Coordination Centre.
