Job summary

Gloucestershire Health and Care NHS Foundation Trust is seeking a Part-time Community Matron / Clinical Case Manager for 28.75 hours per week. The role involves autonomous clinical practice to triage, plan, manage and coordinate complex care for patients at home within Gloucester, Cheltenham and Forest of Dean localities.

The post focuses on proactive case management, health coaching, and collaboration with primary and secondary care to reduce unplanned admissions and improve patient outcomes.

Qualifications

• Registered nurse or Allied Health professional with relevant registration.
• Post-registration qualifications/experience in managing complex long-term conditions and frailty.
• Evidence of postgraduate qualifications in physical assessment and clinical reasoning (PACR).
• Experience in case management models with personalised goals, care planning and self-management.

Responsibilities

• Triage patients to determine service eligibility.
• Conduct holistic assessment in patient home environments.
• Assess safety needs and determine appropriate care settings.
• Review medications for effectiveness and safety; collaborate with prescribers.
• Identify, prioritise and implement case management plans to achieve health gains.
• Maintain timely communication with patients, carers and services.
• Attend and lead MDT meetings as required.
