Job summary

Advanced clinical assessment and prescribing skills expected.
Strong leadership and collaborative working abilities.
Experience in coordinating care for people with complex needs.

Responsibilities

Provide expert assessment, evidence-based treatment and complex case management.
Act as named Community Matron for an Integrated Neighbourhood Team (INT), coordinating care across the network.
Collaborate with community, district nursing, primary/secondary care, mental health, social care and voluntary sector partners.
Contribute to multidisciplinary meetings and high-intensity user forums.
Support people with conditions including COPD, diabetes, Parkinson’s and CHD.
Educate patients and carers for self-management and care across health and social care teams.
Provide clinical leadership and promote safe, high-quality practice.
Monitor care quality and contribute to service development.
