Job summary

Northumbria Healthcare NHS Foundation Trust is seeking a Community Matron (Band 7) to join our Community Nursing Teams across South East Central Northumberland. The post focuses on identifying vulnerable patients, proactive case management and reducing hospital admissions, with care home support.

You will lead clinical teams, supervise staff, and contribute to a culture of compassionate leadership and continuous improvement within a large NHS trust.

Qualifications

Registered Nurse (NMC) with 1st level registration.
First level degree in health-related subject.
Advanced physical assessment and history taking qualification.
Registered mentor status.
Post-registration advanced/specialist qualification preferred.

Responsibilities

Conduct advanced clinical assessment and care planning for complex needs.
Coordinate care with multi-disciplinary teams to reduce admissions.
Provide clinical leadership and supervise nursing staff.
Support staff development through training and supervision.
Promote compassionate leadership and quality improvement.
