Job summary

The post involves working within national guidelines, collaborating with GPs, pharmacists, and other professionals, and attending MDT meetings to drive integrated care for patients in the community.

Qualifications

• Experience in community health or equivalent setting.
• Strong assessment and communication skills.
• Knowledge of local protocols and signposting.

Responsibilities

• Triage process: ensure patients meet the criteria for the service.
• Holistic assessment in a patient’s home environment using advanced and specialised skills.
• Risk assessment to determine the appropriate care setting and signpost or refer where needed.
• Medication assessment and review in collaboration with prescribers to ensure safe prescribing practices.
• Case management to develop, implement and evaluate care plans and maximise patient independence.
• Attend weekly internal MDT meetings and participate in external MDT meetings as required.
