Qualifications

• Understanding of person-centred care, care planning and reviews.
• Full UK driving licence and access to own vehicle.

Responsibilities

• Carry out new client consultations and care assessments.
• Develop, review and audit person-centred care plans.
• Build strong relationships with clients, families and healthcare professionals.
• Mentor and supervise Care Professionals through regular supervisions.
• Conduct client reviews and ensure care adapts to changing needs.
• Maintain digital care records and support care planning systems.
• Identify training and development needs within the care team.
• Participate in on-call rota and support the Registered Care Manager.
