Job summary

Libertatem Healthcare Group is seeking a Senior Care Coordinator in Wokingham to lead and develop a care coordination team, ensuring delivery of person-centred services. The role focuses on operational excellence, team development and financial oversight within a permanent full-time position.

The ideal candidate has at least four years in care coordination or related roles, strong leadership, excellent communication, and proficiency with One Touch and MS Office.

Qualifications

• Minimum of 4 years’ experience in care coordination or related health/social care roles.
• Strong leadership and coaching abilities with a collaborative approach.
• Experience handling payroll, billing and rota validation and related finance tasks.

Responsibilities

• Lead, coach and develop a team of Care Coordinators for high-quality service.
• Oversee rota efficiency, workforce deployment and compliance processes.
• Resolve payroll, billing and timesheet queries with accuracy and improvements.
• Collaborate with Clinical, HR and Operational teams for smooth service delivery.
• Champion best practices in documentation and system usage.
