Job summary

ID Medical in Milton Keynes is seeking a Senior Care Coordinator to manage complex healthcare packages, ensuring safe, person-centred and compliant service delivery.

You will lead care teams, coordinate mobilisation and rosters, monitor KPIs and maintain continuity of care with safeguarding and regulatory duties.

The role requires health or social care management experience, strong communication, stakeholder management and the ability to prioritise competing demands.

Qualifications

• Experience coordinating or managing regulated health or social packages of care.
• Experience of rota management, workforce planning and maintaining continuity of care.
• Experience as a Senior Care Coordinator or in a comparable role, coordinating field-based care teams.

Responsibilities

• Manage complex healthcare packages, ensuring safe, compliant and person-centred care in line with contractual and regulatory requirements.
• Triage operational enquiries, incidents and escalations; monitor risks and implement contingency arrangements to maintain continuity of care.
• Act as the primary contact for clients, families and representatives, building trusted relationships and coordinating communication across teams.
• Coordinate package mobilisation and ongoing changes, ensuring workforce and service readiness.
• Lead rota management and workforce planning, coordinating cover, forecasting staffing needs and escalating workforce risks.
• Work with the Clinical Team to support safe delivery of delegated healthcare tasks in line with care plans, risk assessments and clinical guidance.
• Monitor package performance against KPIs and service standards, taking corrective action and escalating risks to performance or viability.
