Job summary

Libertatem Healthcare Group is seeking a proactive Care Coordinator to ensure high-quality domiciliary care and strong support for our clients and staff.

You will develop care schedules, coordinate with HR and Clinical Leads, mentor team members and maintain accurate records while building relationships with clients and families.

Qualifications

  • Experience in domiciliary care or similar setting.
  • Excellent customer service and communication skills.
  • Strong logistical planning and problem-solving abilities.
  • Calm under pressure with a collaborative mindset.
  • Experience mentoring or coaching team members.
  • Own transport is essential.
  • 25 days annual leave + bank holidays.
  • Birthday off.

Responsibilities

About the Opportunity

We are seeking a proactive and experienced Care Coordinator to join our growing team.
