Job summary

Concept 11 is seeking a dedicated care worker to provide direct care and shift support for individuals with complex needs and challenging behaviours.

You will guide care staff, help coordinate enhanced staffing, monitor care plans and risk assessments, and identify triggers to lead appropriate responses during incidents. This role requires experience in health and social care, safeguarding knowledge, and flexibility for 38–48 hours per week.

Qualifications

• Experience in health and social care supporting individuals with complex needs and challenging behaviours.
• Understanding of safeguarding, duty of care, confidentiality and person-centred care.
• Ability to follow care plans, risk assessments and behavioral support plans.

Responsibilities

• Provide direct care and shift support to individuals with complex needs and challenging behaviours.
• Guide and support care staff, coordinate enhanced staffing arrangements and monitor care plans and risk assessments.
• Identify behavioural triggers and lead appropriate responses during challenging situations; support de-escalation and incident reporting.
